I understand there is a $20 application fee and it is non-refundable. I certify that in the calendar year 2008, I will be between the ages of 16 -25. I understand that by entering the OK City Star Competition for 2008, I give my permission to have my performance publicized in any way deemed appropriate by the organizing committee. I agree to abide by the rules and guidelines of the competition and, if selected as the winner, to conduct myself with professionalism as a representative of this competition in the upcoming year. I will not hold the OK City Star Competition, its organizers or sponsors liable for any injury sustained during the competition.

I understand that my title and winnings may be revoked if I do not abide by the rules and guidelines established above.
